What to do if you or your co-worker is injured on the job...
Report life-threatening injuries to the
nearest security office immediately, who will complete an incident
report. move
victims who are severly injured until properly trained staff
arrives. Security
officers
who are first-aid trained
and have radio equipment for summoning rescue personnel. If you
cannot quickly contact security, call 911 to report the location
of the victim. If possible, have someone stand by to direct rescue
personnel to the victim. The supervisor must complete and fax
a First Report of Injury form within 24 hours of injury. Fax
the
form
to the risk management office at 904.632-3145 within 24 hours.
Report injuries that are not life-threatening
to your supervisor immediately and complete a First Report
of Injury (Florida Department of Labor form [DWC-1]). Forms
are available in security, campus business offices and the human
resources
department.

A supervisor is to call the risk management
department to request referral and pre-authorization to a participating
workers compensation
facility as this is required for non-emergency medical treatment.
If your injury requires medical attention, ask your supervisor
for a referral to a participating facility. Note: The
use of emergency rooms is not authorized except for genuinely
serious
injuries
or those occurring after normal business hours.
Non-emergency treatment must be obtained
from the medical providers listed below in order for the College
to accept financial responsibility.

Pharmacies require approval by risk
managment prior to billing the College for prescribed medicines.
Instruct
them to contact
the risk management office at 904.632.3127 or 904.632.3166 during
business hours for approval. Requests for reimbursement of
pre-paid
medicines or supplies must include original receipts and be forwarded
to the risk management office timely. Tip:
Pavilion (at Baptist Medical Centers) and K-Mart Pharmacies
are
most familiar with the handling of Florida Community College
workers compensation prescriptions.
Injured employees who are cleared by the College’s worker’s
compensation physicians to return to work with defined restrictions
are required
to fax to 904.632.3087 a copy of the physicians return to work
documents and to bring a copy to human resources the same day
(if possible) to explore openings the College may have to accomodate
the physical restrictions.
Treating workers compensation physicians may refer you to specialists
if they determine that such care is needed. You may accept the
referral or choose a specialist from the authorized panel or
list of participating specialists (maintained in the risk management
office).
