Vendor Application

Vendors interested in doing business with the Florida Community College at Jacksonville must complete both a vendor application form and the IRS W-9 form. These application forms can be downloaded below from this Web page, or you can contact Patty Carter at 904.632.3303 and request an application and W-9 form be mailed to you.

Please be aware, FCCJ also posts formal solicitations to the Vendor Bid System (VBS) managed by the Department of Management Services (DMS). At this location, vendors can register (at no cost) to download copies of bid/proposal solicitations statewide.
