Benefit Programs and Requirements

All forms necessary for application for educational benefits are available at the financial aid/enrollment services office. Application requirements for educational benefits vary from program to program. Your application package must be complete.

Requirements for each program's initial application follow:

Chapter 30 — Montgomery GI Bill


This benefit is for those veterans who served on an active duty after June 30, 1985 (Category 1), those who had remaining entitlement under the Vietnam Era GI Bill (Category II), and those who switched from VEAP before being involuntarily separated after February 2, 1991 (Category III).

Category I & III — application package includes:

  1. Florida State College at Jacksonville admission approval
  2. Form 22-1990 Application for Educational Benefits
  3. Certified DD214's (Member Copy 4)
  4. Statement of Understanding — a form stating you have read and understood the veteran’s benefit process.
Category II (commonly called Chapter 34/30) — application package includes:
  1. Florida State College at Jacksonville admission approval
  2. Form 22-1990 Application for Educational Benefits
  3. Form 21-686C Declaration of Status of Dependents
  4. Certified copies of ALL DD214’s
  5. Delayed Entry Contract - for those who signed a delayed entry contract before January 1, 1977 and began their active duty experience prior to January 2, 1978.
  6. Form 21-674 for dependent children over age 18 but still enrolled in either high school or college.
  7. Statement of Understanding

Chapter 31 — Vocational Rehabilitation


This benefit is designed to assist veterans with a service-connected disability in obtaining and maintaining employment. A service-connected disability rating is required as part of the eligibility requirements.

Veterans should apply for vocational rehabilitation through Vocational Rehabilitation and Employment, 7825 Baymeadows Way, Suite 120B, Jacksonville, FL 32256, 904.731.0565. They will receive guidance from the Vocational Rehabilitation and Employment Division of the DVA regarding application and admission requirements.

Chapter 32 — VEAP/Veterans Educational Assistance Program

This benefit is for those veterans who served between the Vietnam GI Bill Era and the Montgomery GI Bill era. Generally, between January 1977 and June 1985.

The application package includes:
  1. Florida State College at Jacksonville admission approval (as outlined on Page 1 of this handbook). Form 22-1990 Application for Educational Benefits
  2. Certified DD214’s
  3. Statement of Understanding

Chapter 35 — Survivor’s and Dependent's Educational Assistance


This benefit is for surviving spouses and children of veterans who died of a service-connected death, surviving spouses and children of veterans who died as a result of a service-connected disability, or for surviving spouses and children of a veteran with a permanent service-connected disability.

The application package includes:
  1. Florida State College at Jacksonville admission approval
  2. Form 22-5490 Application for Survivor's and Dependent's Educational Assistance
  3. Provide proof of relation to veteran (marriage certificate, birth certificate or adoption decree).
  4. Statement of Understanding
Chapter 1606 — Reservist Benefit

This benefit is for those currently serving in the Selected Reserves.

The application package includes:
  1. Florida State College at Jacksonville admission approval
  2. Form 22-1990 Application for Educational Assistance
  3. DOD Form 2384 Notice of Basic Eligibility (NOBE). This form is issued through the Reserve Unit
  4. Statement of Understanding
Note: Other types of documentation (i.e. birth certificates) are also required in order to be certified - true copies or originals.
