Field Experience Forms and Resources

There is quite a bit of important information and forms you’ll need as part of the field experience process.

The Field Experience Handbook — Your starting point and reference guide

Cooperating Teacher Letter — Some general information for the teacher with whom you’ll be working. Please give this form to your cooperating teacher at the school where you observe.

Field Experience Log — To record the activities that were part of your experience. Please give this to your instructor after completing field experience.

Student Evaluation — For the teacher with whom you worked. Please ask your cooperating teacher to give this to your instructor, or carry it to your instructor from the cooperating teacher in a sealed envelope.

Placement Site Questionnaire — A chance for you to evaluate the school and your experience. Please send this to the Field Experience Facilitator. Her address is on the form.

Community Agencies Reference List — Options for EDG 2701 only students who plan to do their field experience at a community agency instead of a school.
